Ambulance (EMS) Reports

 Ambulance Reports are released to the patient, their legal representative or by Subpoena.

 If you need a copy of your ambulance report please Contact Paramedic Billing Services at 630-530-2988 or www.paramedicbilling.com. They will send you a HIPPA release form.  

If you need to update your health insurance information you may also contact Paramedic Billing Services.

 We utilize a Third-Party Billing company in Elmhurst, Paramedic Billing Services for all Ambulance, MVC and Hazardous Material related billing.

Please fill out the FOIA Form and return to obtain a copy of your fire report. 

 Under Illinois State law, the Fire Chief is responsible for investigating all fires within our District. Contact the us at 847-455-0180 to request an investigation report.
